Abstract
Pain is a common and devastating symptom of cancer affecting patients lives sometimes more than the cancer itself. Despite the pharmacological and non pharmacological methods available to manage pain, pain is still a felt and remains a devastating factor affecting cancer patient.
This study aimed at investigating nurses knowledge on the management of cancer pain in the Buea regional hospital, the specific objectives being: to investigate nurses knowledge on cancer pain, to asses nurses knowledge on the management of cancer pain and to explore challenges faced by nurses in the management of cancer pain.
It was a cross sectional study design with a sampling size of 100 participants using a convenient random sampling technique. According to the results obtained, 85% of the participants have knowledge on what cancer pain as it is pain that is experience by patients that are suffering from cancer either at the early stage or the advanced stage of the disease, 75% of the participants have good knowledge on how cancer pain can be managed and 100% of the nurses knows the various challenges faced in the management of cancer pain.
Conclusively, the participant have knowledge on how cancer pain can be manage though there is still the need to crate an oncology center in the hospital for proper management of the patients as the case, the patients are only kept for few days and then referred to the hospital that has adequate medical facilities. Hence it was recommended that the ministry of public health should an oncology center and more trained healthcare personnels should be employed.
CHAPTER ONE
INTRODUCTION
This chapter runs from the background, statement of problem, general objectives specific objectives, research questions and operational definition of terms.
1.1 Background Of Study
Cancer is among the leading cause of morbidity and mortality worldwide, responsible for about 18.1million new cases and 9.6million death in 2018( W H O, 2018). Cancer related pain is a serious health problem throughout the world. Infact, Cancer is a prevalent disease and the majority of cancer patients experience pain throughout their illness( Foley 2000).
Cancer pain can result from the primary and mestastic site or from treatment related squel. Despite pain assessment and treatment, pain remain suboptimal (Gordon et al, 2005). Breakthrough pain was shown to be common with only minority of patient prescribe on appropriate dose of rescue medication (Brevik et al, 2009).
Pain according to the International Association for the Study of Pain (IASP) is unpleasant sensory and emotional experience associated with or potential tissue damage or described in terms of such damage” (Merskey and Bogduk, 1994).
Cancer patients currently live longer than previously because of earlier diagnosis and developments in treatment procedures and cancer and its treatment can be painful (Portenoy and Lesage, 1999). Therefore, cancer patients are expected to experience pain. Although guidelines and pharmacological interventions exist to manage cancer pain, poor assessment and under-medication is well-documented in the UK, US, Canada, France, and Asia (Breivik et al., 2009).
More than 14million cases of cancer were diagnosed worldwide in 2012 and by 2025, the number is expected to reach more than 20million (Department of hematology and medical Oncology, Fox Chase Cancer Center advances in understanding early detection and treatment of cancer, progress related to treatment of cancer pain has been slowed and largely inadequate. Increase guideline for treatment and increase worldwide consumption of opioids (albeit with tremendous regional variability), have helped to reduce the burden of cancer
pain.
However, the prevalence of cancer pain remains high and continues to be one of the most feared aspect of the disease. Cancer is the second leading cause of death globally, and responsible for an estimated 9.6million deaths in 2018. Globally, about 1 in 6 deaths is due to cancer.
Approximately 70percent of deaths from cancer in low and middle income countries because of the aging and growth of the population as well as increased prevalence of risk factors associated with economic transition including smoking, alcohol, obesity, physical inactivity, and reproductive behaviors), and of certain infectious agents of importance in cancer etiology (WHO) Pain is a common symptom among cancer patients, affecting the life style of the patient. 69% of cancer patients have reported that pain restricts their daily life activities (Breivik et al., 2009; Lee et al., 2014).
The step- wise treatment, established by World Health Organization (WHO) in 1986, has been aimed to treat pain effectively. (Burton and Cleelan., 2001) This protocol reduces pain in 70 to 90% of patients when administered optimally. (Van den Beuken-van Everdingen et al., 2007)
However, prevalence studies performed have demonstrated that cancer pain still affects 56 to 64% of patients. (Van den Beuken-van Everdingen et al., 2007; Breivik et al., 2009, Budkaew and Chumworathayi, 2013). Moreover, it has been reported that 43% of patients on therapy received insufficient treatment (Deandrea et al., 2008).
Therefore, the healthcare providers should have sufficient knowledge on cancer pain. In Africa, the prevalence of cancer pain ranges from 35.7% to 87.5% with the prevalence being more than 70% in most of the studies ( M. Abdel-Wahab, N. EL-Gbawalby, M. Mostafa et al., 2007 ).
In Cameroon from 1986-1991, about 1000 cases of cancer were diagnosed histologically every year. During this period, investigations showed that only 1 cancer out of 10 is diagnosed histologically. Thus nearly 10000 new cases of cancer are expected yearly in Cameroon where a population of 12000000 inhabitants is found leading an incidence of about 100/100,000. Forty-nine percent of patients are females and 51% males.
Twelve percent of patients are less than 20years old and 60% are between 40 and 70 years. Liver cancer is at the top of the list if cancer, representing 20% followed by skin cancer 15%, breast cancer 11%, and cancer of the cervix 11%. For men, liver cancer is the most frequent while cancers of the breast and uterine predominate in women. A Mbakop et al.; 1992
This study on investigating nurses knowledge on the management of cancer pain in the Buea regional hospital south west region of Cameroon, it participants are nurses working in the Buea regional hospital. This study was carried out from November 2020 to April 2021.
1.2 Statement Of Problem
The International Association for the Study of Pain ( IASP ) defines pain as an unpleasant sensory and emotional experience associated with actual or potential tissue demage. Pain is a common and devastating symptom of cancer affecting patients lives sometimes more than the cancer itself.
Despite the pharmacological (the use of drugs) and non pharmacological ( for example massage) methods available to manage pain, pain is still a felt and remains a devastating factor affecting cancer patients.
Also from observation by the researcher during clinical experience, she observed that patients suffering from cancer experience pain despite the administration of pain relieving drugs and from personal experience, the death of an uncle who suffered and died of cancer. These prompted the researcher to carry out the research.
